About The Position

This role is on the Unsheltered Homelessness Response Pilot (UHRP) Team. The primary focus is to deliver housing-focused, intensive case management services to support pilot participants in obtaining and maintaining permanent housing, building long-term stability, and preparing for sustained success beyond program exit. Collaboration with housing providers, peer support services, and other agencies will be essential to provide holistic care.

Requirements

  • Proficiency in working with client care databases.
  • Minimum of two years of sustained sobriety if in active recovery.
  • Strong knowledge of community resources and navigation skills.

  • Familiarity with ethical guidelines in trauma-informed care and outreach practices.
  • Must follow culturally adapted methods for diverse populations.
  • Strong organizational skills with the ability to balance multiple responsibilities and manage caseloads effectively.
  • Empathetic, trustworthy, and non-judgmental when working with unsheltered individuals, utilizing trauma-informed approaches.
  • Excellent communication skills for building trust with clients, community partners, and staff members.
  • Ability to adapt to changing environments and circumstances with creativity and humility.
  • Skilled at recognizing and responding to crises, emergencies, and risks that affect client welfare and safety.
  • Willingness to collaborate across departments, agencies, and community partners to provide comprehensive care.
  • Respect for and understanding of diverse cultural backgrounds, with a commitment to providing inclusive services.
  • Actively promote peer support models, integrating peer support into outreach and case management activities.
  • Proficiency in Microsoft Office (Excel, Outlook, Word) and client management databases is required.
  • Ability to read, analyze, and interpret common documents.
  • Ability to respond to common inquiries or complaints from participants, or members of the community.
  • Ability to effectively present information to top management and public groups.

Nice To Haves

  • At least two years of experience working with individuals experiencing homelessness, SUD, SMI, or SPMI.
  • Experience in case management or a related field.
  • Experience with street outreach.
  • Associate’s degree in social work, human services, or a related field and/or certification as a CSAC or LCAS; Bachelor’s degree in social work or related field preferred.

Responsibilities

  • Maintain active engagement with assigned participants according to acuity-based service standards.
  • Conduct in-person visits, phone check-ins, and outreach to ensure participants remain connected to the program.
  • Respond to participant crises promptly, collaborating with peer staff and external providers as needed.
  • Provide housing-focused case management, including support with applications, denial appeals, reasonable accommodation requests, and landlord advocacy.
  • Develop, update, and monitor Housing Stability Plans, ensuring participant progress toward stated goals and permanent housing.
  • Assist participants in overcoming barriers to housing, including documentation, benefits navigation, and service coordination.
  • Work closely with CPSSs to coordinate services, outreach, and crisis response.
  • Participate consistently in case conferencing, team meetings, and internal coordination to align participant support.
  • Collaborate with external partners to facilitate housing placements and services.
  • Maintain accurate, complete, and timely records of participant interactions, services provided, and progress in the program database.
  • Ensure all updates and key documents (leases, housing plans, service records) are uploaded and current.
  • Provide sufficient documentation to support service delivery, monitoring, and reimbursement requirements.
  • Participate in pilot events, outreach activities, and other program-related initiatives.
  • Support the program in achieving its housing and engagement outcomes through consistent, accountable work aligned with the pilot’s goals.
