About The Position

Summit Home Care and Hospice is seeking an experienced and motivated STNA to join their Home Health & Hospice teams. The company emphasizes providing personal, compassionate healthcare delivered at home. The STNA will work under the supervision of a Registered Nurse or therapist, performing personal care services, observing and documenting patient conditions, and assisting with various daily living activities. The role involves travel within a ~30-mile radius for patient visits, with mileage reimbursement provided. AI-assisted clinical documentation is available to streamline charting.

Requirements

  • Must meet the training and licenses requirements in accordance with state and federal laws of STNA.
  • Must be at least 18 years of age.
  • Must be able to read and follow written instructions and document care that is given.
  • Self-directing with the ability to work with little direct supervision.
  • Empathy for the needs of the ill, injured, frail and impaired.
  • Demonstrates tact, patience, and good personal hygiene.

Responsibilities

  • Perform various personal care services as necessary to meet the patient's needs.
  • Observe patients, report these observations, and document observations and care performed.
  • Provide personal care including bathing, assisting with dressing and undressing, toileting, oral hygiene, and skin care.
  • Change bed linens and assist with other laundry as appropriate.
  • Maintain a clean and orderly living area for patients, as needed.
  • Plan and prepare nutritious meals and assist in feeding the patient if necessary.
  • Take and record vital signs (temperature, pulse, respiration, blood pressure) when ordered.
  • Assist in ambulation and exercise according to the plan of care.
  • Perform range of motion and other simple procedures as an extensional therapy service as ordered.
  • Assist patients in the self-administration of medication.
  • Meet the safety needs of patients and use equipment safely and properly.
  • Report on patient's condition and significant changes to the assigned nurse.
  • Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.
  • May care for patients in home care and hospice departments as the census/caseload allows.
