About The Position
Summit Home Care and Hospice is seeking an experienced and motivated STNA to join their Home Health & Hospice teams. The company emphasizes providing personal, compassionate healthcare delivered at home. The STNA will work under the supervision of a Registered Nurse or therapist, performing personal care services, observing and documenting patient conditions, and assisting with various daily living activities. The role involves travel within a ~30-mile radius for patient visits, with mileage reimbursement provided. AI-assisted clinical documentation is available to streamline charting.
Requirements
- Must meet the training and licenses requirements in accordance with state and federal laws of STNA.
- Must be at least 18 years of age.
- Must be able to read and follow written instructions and document care that is given.
- Self-directing with the ability to work with little direct supervision.
- Empathy for the needs of the ill, injured, frail and impaired.
- Demonstrates tact, patience, and good personal hygiene.
Responsibilities
- Perform various personal care services as necessary to meet the patient's needs.
- Observe patients, report these observations, and document observations and care performed.
- Provide personal care including bathing, assisting with dressing and undressing, toileting, oral hygiene, and skin care.
- Change bed linens and assist with other laundry as appropriate.
- Maintain a clean and orderly living area for patients, as needed.
- Plan and prepare nutritious meals and assist in feeding the patient if necessary.
- Take and record vital signs (temperature, pulse, respiration, blood pressure) when ordered.
- Assist in ambulation and exercise according to the plan of care.
- Perform range of motion and other simple procedures as an extensional therapy service as ordered.
- Assist patients in the self-administration of medication.
- Meet the safety needs of patients and use equipment safely and properly.
- Report on patient's condition and significant changes to the assigned nurse.
- Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.
- May care for patients in home care and hospice departments as the census/caseload allows.
