About The Position

This position at Cleveland Clinic Justin T. Roger Hospice Care Center involves helping patients and their families physically, emotionally, and spiritually prepare for the end of life. Caregivers work alongside compassionate physicians, nurses, and other healthcare staff to offer support and comfort as patients adjust to the hospice environment. The focus is on providing comfort, not a cure, and quality of life, not duration. Every day, caregivers can make a lasting difference in the lives of both patients and their loved ones. This is a PRN (Pro Re Nata) position, meaning a caregiver will work varying hours and shifts. Requirements

  • High School Diploma or GED
  • State Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse’s Aide Registry
  • One year of clinical experience as an Aide in an acute, rehab or skilled nursing setting
  • Valid Driver’s License (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross within orientation period

Nice To Haves

  • Hospice and Palliative Care Nurse Assistant (CHPNA) Certification
  • Home Care experience

Responsibilities

  • Perform basic patient care activities.
  • Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate.
  • Assist patients with tending to personal care, activities of daily living and transfers/transport.
  • Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff.
  • Assist with keeping the patient's environment clean and orderly.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Escalate change in patient condition to patient’s physician as appropriate.
