Requirements

  • Must be able to read and write English proficiently.
  • Must be a state-tested nursing assistant in good standing.
  • Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.
  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation.
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed.
  • Provide an active driver’s license.
  • Ability to provide proof of eligibility to work in the United States.
  • High level of integrity, ethics and professionalism.

Responsibilities

  • Provide personal care and assist with ADL's according to the POC.
  • Provide a safe environment for patients.
  • Safely ambulate/transfer patient as appropriate.
  • Assist with passive and active ROM exercises as directed by the nurse.
  • Prepare nutritional snacks as appropriate for the patient.
  • Under the supervision of the nurse, assist patient with pre-poured medications that are ordinarily self-administered and that are prescribed by a physician under the supervision of the nurse (Home Care only).
  • Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
  • Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
  • Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
  • Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
  • Attend and actively contribute during patient care conference.
  • Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
  • Attend in-services to obtain 3 education hours per quarter.
  • Participate in the on-call rotation as scheduled (Home Care only).
  • Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.).
  • Demonstrate positive customer-service responsiveness according to agency standards.
  • Flexible in meeting patient/family needs that require variable shifts and weekends.
  • Participate in chart review quality improvement activities and other meetings as assigned.
  • Participate in coordination of and communication with volunteer staff.
  • Provide basic care-giving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens.
  • Maintain Transmission Based Precaution.
  • Complete annual competency requirements.
