About The Position

As a Home Health Aide (HHA), the work you do every day makes a difference in the lives of our patients. As a Home Health Aide (HHA) you are an essential member of the home care team who works under the supervision of a nurse. You provide compassionate support with activities of daily living and basic personal care needs while ensuring a safe, comfortable living environment. Interacting with patients' families while caring for your patients and experiencing the rewarding privilege to be part of every step of their recovery journey.

Requirements

  • High school diploma or equivalent.
  • State Tested Nurse Aide (STNA) or 75-hour nurse aide training (CNA).
  • 1 year of experience as a nurse aide in a hospital, nursing home, home care agency, or a private setting under the direct supervision of a RN, which can be verified.
  • Completion of skills competency test with 80% or higher score.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.

Responsibilities

  • Perform personal care including but not limited to: bathing, grooming, skin care, toileting, and oral hygiene.
  • Effectively communicate and document any changes in patient conditions to the registered nurse or supervisor and maintain positive relationships with your patient and team.
  • Be responsible for all practices and duties within the home health aide scope of practice as outlined by the state for its HHA personnel.
  • Assist patients as needed with lifting and transferring.
  • Maintain good relationships with the patient, the patient’s family, and staff.
  • Report any changes to the patient’s medical and living conditions to the supervisor or the case manager.
  • Ensure the living environment is healthy and safe.
  • Complete all documentation following agency protocol and Medicare/Federal guidelines.
  • Attend in-service training and mandatory agency meetings.
