About The Position

Avalon Housing is a non-profit supportive housing provider dedicated to the mission that housing is a human right. They offer supportive housing, which combines affordable housing with services like case management, to help individuals find, maintain, and thrive in their homes. Avalon is committed to creating diverse, inclusive, and equitable communities. The State Crossing Support Coordinator role is based at a new apartment building with over 30 units and involves providing on-site support, case management, and coordinating services with community partners. The coordinator will engage clients creatively, build trust, maintain documentation, and respond to crises or emergencies, potentially involving contact with emergency response systems. While primarily working on-site at State Crossing, there are some remote working opportunities. The position is part of the Outreach Team but focused on the State Crossing location.

Requirements

  • At least three years experience in human services, preferably working with populations who are experiencing homelessness, mental health disorders, chronic health conditions, and/or substance use disorders
  • Capacity to respond to crisis situations with effective use of de-escalation skills
  • Strong communication skills – both verbal and written
  • Strong problem-solving skills
  • Strong documentation skills; HMIS experience preferred
  • Ability to collaborate and maintain community partnerships
  • A valid driver's license and reliable transportation is required
  • Demonstrate the ability to work well with diverse populations in a non-judgmental way

Responsibilities

  • Provide ongoing, active outreach and creative engagement to tenants
  • Conduct comprehensive assessments and help tenants develop action plans to achieve goals
  • Work with tenants and property management to coordinate eviction prevention efforts and develop housing permanency plans
  • Negotiate and advocate with external community resources providing medical, mental health, substance use, and other supports
  • Assist tenants with conflict resolution among tenants and neighbors
  • Work to build community and peer support among tenants, including participation in community programming activities
  • Meet documentation requirements as dictated by program need
  • Work with clients in their homes, in community centers, and in the larger community
  • Participate in on-call rotation and occasional evening and weekend work as necessary
  • Assist tenants as needed with activities of daily living such as transportation, housekeeping, meal preparation, medication, shopping, laundry, or other hands-on assistance
