About The Position

Home Heart Florida is a pediatric home health agency supporting children with complex medical needs at home. The Start of Care Nurse completes initial home evaluations, establishes individualized care plans, and supports families in beginning a safe, high-quality home health experience. The role serves as an important point of contact for families and collaborates with the clinical team and coordinating nurse. The region is Tampa, with visits including Start of Care, Resumption of Care, Recertification, and FHHA Supervisory Visits. English is preferred; Spanish is a plus.

Requirements

• Active Florida RN license and BLS certification.
• At least 1 year of experience completing Start of Care visits in home health.
• Strong clinical documentation and EMR experience.
• Valid driver’s license and reliable transportation, with willingness to travel to patient homes.
• Comfort working with medically complex pediatric patients.

Nice To Haves

• Bilingual English/Spanish skills.
• Experience with Statewise EMR.
• Pediatric home health or private duty nursing experience.

Responsibilities

• Conduct comprehensive start-of-care assessments in patients’ homes.
• Develop and initiate individualized care plans based on physician orders.
• Educate family caregivers about care needs, equipment such as ventilators, G-tubes, and pumps, medications, and safety.
• Complete thorough and accurate clinical documentation.
• Communicate findings and care plans to the clinical team and coordinating nurse.
