Start of Care (SOC) Nurse
About The Position
Home Heart Florida is a pediatric home health agency serving children with complex medical needs. The Start of Care Nurse completes initial home evaluations, establishes care plans, and helps create a safe, high-quality home health experience for patients and families. The role serves as an important touchpoint for families at the beginning of their care journey. Visits are in patients’ homes in the Daytona Beach / Port Orange region, covering east Volusia down to New Smyrna Beach and Edgewater. Visit types include Start of Care, Resumption of Care, Recertification, and FHHA Supervisory Visits. The schedule is part time or per diem.
Requirements
- Active Florida RN license and BLS certification.
- At least 1 year of experience completing Start of Care visits in home health.
- Strong clinical documentation and EMR experience.
- Valid driver’s license and reliable transportation, with willingness to travel to patient homes.
- Comfort working with medically complex pediatric patients.
Nice To Haves
- Experience with Statewise EMR.
- Pediatric home health or private duty nursing experience.
Responsibilities
- Conduct comprehensive Start of Care assessments in patients’ homes.
- Develop and initiate individualized care plans based on physician orders.
- Educate family caregivers about care needs, equipment such as vents, g-tubes, and pumps, medications, and safety.
- Complete thorough and accurate clinical documentation.
- Communicate findings and care plans to the clinical team and coordinating nurse.
