About The Position

This role involves visits to homes, to participants of the LIFE program. The organization offers mileage reimbursement for distances driven.

Requirements

  • Associate's degree or graduate from an accredited school of nursing.
  • One year working with the frail elderly population.
  • AHA BLS - Healthcare Provider
  • Drivers License
  • Registered Nurse – NJ or Registered Nurse – NLC multi-state

Nice To Haves

  • BSN preferred.
  • One year home care experience preferred.

Responsibilities

  • Deliver quality care to patients through a team effort in working with others through the nursing process of assessment, diagnosis, planning, implementation and evaluation.
  • Assure patient safety by executing appropriate policies & procedures.
  • Perform routine nursing assessments, triage, medication administration, wound care, treatment and participant education.
  • Conduct comprehensive nursing assessments in home and clinic settings, at appropriate intervals, at times of status changes and as needed with each participant, family members and care givers.
  • Collaborate with other members of the PACE care team regarding care delivered to participants in the appropriate care setting through verbal and electronic communication.
  • Order and coordinate DME and medical supplies for multiple participants to support ongoing care needs in the home.
  • Participate in Interdisciplinary Team (IDT) meetings, case reviews and care planning discussions
  • Mobilize resources in complex cases to maximize patients control and participation over his/her own recovery.
  • Provide information and interpretation of the participant’s condition, treatment(s), and offers coping mechanisms. Provide these to both participants and family/care givers.
  • Integrate assessment and diagnostic information with intuition to foresee potential age-specific healthcare needs. Anticipates participant needs.
  • Use discretionary judgment to appropriately modify participant care regimens. Facilitates appropriate response from other health care team members to provide quality and safe care.
  • Explore multiple aspects of care regimen with case manager and health care clinicians to promote appropriate resolution.
  • Participate in performance improvement activities at the department and/or hospital wide level.
  • Participate actively in the preparation of various regulatory agency readiness.
  • Performs any other related duties as required or assigned.
