About The Position

This is a full-time, 40 hours per week, dayshift position for experienced RNs. The role is a triage position where the RN will work with a group of physicians, fellows, and NPs to provide care coordination for patients. Responsibilities include communicating with patients, families, and ancillary services via Epic telephone/My Chart encounters and phone. Care coordination activities involve addressing medical issues/concerns, communicating test results, managing medication refills/prior authorizations, providing patient/family education, and liaising with ancillary services such as dieticians, home care companies, insurance companies, and pharmacies. The primary goal is to assist patients in moving through their episode of care efficiently and safely, while maintaining quality. The RN will delegate, direct, oversee, and provide developmentally appropriate patient care, practicing professional nursing and promoting patient advocacy in accordance with the ANA Code of Ethics for Nurses. A current Ohio Nursing License is required.

Requirements

  • BSN or Associate degree with commitment to obtain BSN within 5 years of hire, required.
  • Licensed as a Registered Nurse in the State of Ohio.
  • Current CPR certification required.
  • Must be willing to obtain and maintain NRP, PALS, ACLS, APHON or other necessary certification as determined by your manager based on business unit.
  • Looking for experienced RNs.

Responsibilities

  • Provides nursing care using a Family Centered approach.
  • Assesses individualized patient needs.
  • Communicates patient care needs and priorities to assigned Licensed Practical Nurses (LPNs), Patient Care Assistants (PCAs), Unit Coordinators, and Allied Health personnel.
  • Plans, implements, educates and evaluates care using Evidence-Based Practices.
  • Utilizes Therapeutic Communication while monitoring the physical, psychological, and social needs of patients and families.
  • Engages in the “workplace community” by staying aware of changes, participating in data collection or auditing, understanding Quality Improvement, and representing staff at relevant meetings and then reporting important information and updates back to staff.
  • Documents according to organizational standards, policies, and procedures.
  • Addresses medical issues/concerns.
  • Communicates test results.
  • Manages medication refills/prior authorizations.
  • Provides patient/family education.
  • Communicates with ancillary services such as dieticians, home care companies, insurance companies, pharmacies, and others.
