Provides direct and indirect nursing care in a professional manner based on the Physician Plan of Treatment and the Nursing Care Plan. This role involves clinical case management of homecare enteral patients, including managing authorizations, coordinating referrals and training, performing enteral set-ups and education, reviewing physician orders, updating patient records, evaluating supply usage, ensuring proper kits are provided, participating in billing, monitoring patient response to therapy, and communicating with healthcare providers and the HH-Pharmacy Manager. The position also supports the enteral team and Home Health Nursing Department with education on enteral products, pumps, and therapies, and ensures appropriate patient teaching materials are available. The RN completes discharge summaries and instructions, conducts age-specific assessments, demonstrates clinical competence, and implements nursing actions according to the plan of care. They also assess and educate patients/caregivers, monitor and document responses to treatment, perform reassessments, and know emergency preparedness plans. Documentation must be accurate, timely, and follow company policies and regulatory requirements. The RN develops, implements, and evaluates individualized patient/family nursing actions, formulates goals, and oversees the nursing and interdisciplinary plan of care. Additionally, this role provides direct and indirect work supervision of the Enteral LPN, including training and serving as a resource expert. Regular attendance and performing physical requirements are essential functions. The RN must check emails daily and accept other assigned tasks professionally.
