About The Position
Seeking a Registered Nurse with Home Care Home Base experience and a background in Home Health to provide care in Nelson, Spencer, and southern Bullett counties. Must manage approximately 30 adult patients with a daily target of 6.5 points. No clinical involvement; profiles are directly reviewed by the manager.
Requirements
- Must have Home Care Home Base experience and background in Home Health.
- Required to be licensed in the state of Kentucky.
- Must have at least 1 year of experience on a MedSurg floor in a hospital setting.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) is required.
Responsibilities
- Conduct start of care, recertifications, and daily visits.
- Manage roughly 30 adult patients, case managing with a point system targeting 6.5 points per day.
- Attend to weekend and holiday shifts as required in the rotation schedule.
