The Stabilization Case Manager will support families exiting the Hildebrand Emergency Shelter for 24 to 36 months. This role involves developing initial stabilization plans, maintaining regular communication with families and landlords, documenting progress in ETO/ASIST, and connecting families with necessary public and private resources to ensure housing retention. The Case Manager will also monitor educational progress, assist with accessing medical services, and facilitate engagement with homeowner programs and alumni initiatives. A key aspect of the role is ensuring a high percentage of families maintain stable housing post-placement and advocating for families in housing court. If necessary, the Case Manager will facilitate the transfer of services to other agencies.
