About The Position

Recovery Resource Council is seeking a conscientious, detail-oriented, and compassionate SSVF Healthcare Navigator (HCN) to connect Veterans with VA healthcare benefits or community health services. This role involves providing case management and care coordination, working closely with the Veteran’s primary care provider and interdisciplinary treatment team. The HCN will conduct assessments to understand client situations, barriers to care, and their impact, focusing on community services, outreach, and referrals. The role also includes developing care plans, coordinating services from other organizations, evaluating resource effectiveness, maintaining documentation, communicating with treatment teams, supporting veteran health goals, and providing health education. The HCN must interact with Veterans from diverse backgrounds, upholding the agency's commitment to unconditional acceptance, respect, and positive regard. This is a hybrid position requiring both remote and in-person work, involving sitting, standing, and frequent long-distance driving.

Requirements

  • Bachelor’s degree in a related field.
  • Reliable transportation.
  • Current Texas driver’s license.
  • Evidence of auto liability insurance.
  • Ability to work one on one or with large groups.
  • Ability to work independently and as part of an interdisciplinary team.
  • Ability to plan, organize, set objectives, time frames, and prioritize.
  • Skill in conducting client assessments.
  • Skill in formulating client goals and objectives.
  • Ability to write clear, concise case notes.
  • Strong communication skills both orally and in writing.
  • Knowledge of substance use disorders and mental health issues.
  • Knowledge of community resources.
  • Ability to work flexible schedules.
  • Knowledge of community resources and ability to network.
  • Understanding of and willingness to practice trauma informed care.

Nice To Haves

  • Master level social worker or equivalent education preferred.
  • Experience working with people experiencing homelessness and/or veterans is preferred.
  • Experience working with veterans is a plus.

Responsibilities

  • Conducting assessments with Veteran clients to understand their situation, potential barriers to care, the causes, and the impact of barriers on the Veteran’s ability to access and maintain health care services, as well as their strengths, limitations, risk factors, and supports.
  • Focusing on community services, outreach, and referrals while also working with the interdisciplinary team to develop a care plan.
  • Providing care coordination and connection with services from other organizations and programs to assure services are complementary and comprehensive.
  • Continually evaluating the effectiveness of resources and referrals and making modifications as needed.
  • Maintaining thorough documentation and remaining in communication with the treatment team members when appropriate.
  • Supporting the veteran to meet their health goals and providing ongoing follow-up.
  • Providing health education and materials to the Veteran and family members.
  • Working with and interacting with Veterans from a variety of ethnic, cultural, and economic backgrounds, adhering to the agency guarantee to provide everyone with “unconditional acceptance, respect, and positive regard.”
