Sr. Addiction Counselor
About The Position
Provide comprehensive assessments, evaluations, and diagnostic summaries of chemically dependent patients. Compile Patient centered, comprehensive treatment plans, meeting the regulatory time frames. Document Pt. progress towards the goals of treatment within OASAS regulatory standards. Conduct education and therapeutic treatment protocols focused on Pt. need and OASAS regulations. Provide case management services, in behalf of Pt. with all collateral agencies. Provide necessary reports to TASC, Drug Court, Parole, DCMH, DSS, CPS. Complete discharge summaries in the OASAS time frames, with appropriate aftercare in place.
Requirements
- Bi-lingual in Spanish required
