About The Position

At Concordia Visiting Nurses, we believe care goes beyond the physical. We are looking for a compassionate and faith-driven PART-TIME Spiritual Care Coordinator to provide emotional and spiritual support to patients and families during some of life’s most meaningful and vulnerable moments. Requirements

  • Experience in spiritual care, pastoral care, ministry, chaplaincy, hospice, home health, or a related setting
  • A compassionate and patient-centered approach
  • The ability to provide support to patients and families from diverse backgrounds and belief systems
  • Strong communication and documentation skills
  • The ability to work independently while collaborating with an interdisciplinary team
  • A valid driver’s license and reliable transportation for travel within the service area

Responsibilities

  • Provide spiritual support and counseling to patients, families, and caregivers
  • Assess spiritual needs and help incorporate them into the patient’s overall plan of care
  • Offer comfort, guidance, and emotional support during illness, decline, grief, and end-of-life care
  • Collaborate with nurses, social workers, aides, and other members of the interdisciplinary team
  • Participate in care planning and team meetings
