About The Position

Concordia Visiting Nurses is seeking a compassionate and faith-driven PART-TIME Spiritual Care Coordinator to provide emotional and spiritual support to patients and families during some of life’s most meaningful and vulnerable moments. This role involves walking alongside others with empathy, dignity, and grace, making a real difference in the lives of patients and families. The position offers a supportive team environment, autonomy in the role, and the opportunity to serve with purpose within a mission-driven organization that values dignity, compassion, and whole-person care.

Requirements

Experience in spiritual care, pastoral care, ministry, chaplaincy, hospice, home health, or a related setting.
A compassionate and patient-centered approach.
The ability to provide support to patients and families from diverse backgrounds and belief systems.
Strong communication and documentation skills.
The ability to work independently while collaborating with an interdisciplinary team.
A valid driver’s license and reliable transportation for travel within the service area.

Responsibilities

Provide spiritual support and counseling to patients, families, and caregivers.
Assess spiritual needs and help incorporate them into the patient’s overall plan of care.
Offer comfort, guidance, and emotional support during illness, decline, grief, and end-of-life care.
Collaborate with nurses, social workers, aides, and other members of the interdisciplinary team.
Participate in care planning and team meetings.
