About The Position

Jefferson Healthcare is seeking a compassionate, empathetic, energetic and skilled individual to join our Hospice and Palliative Care team as a Spiritual Care Bereavement Coordinator. This rewarding role requires you to provide spiritual and emotional support to our hospice patients and their families during a difficult time. As Spiritual Care Bereavement Coordinator, you'll be responsible for utilizing compassionate listening with our patients and their families, documenting all visits in EMR, and providing personalized support to help them through their journey. You will conduct interviews, consultations, and emotional, psychosocial, bereavement, and grief evaluations while working closely with mental health providers as well as community spiritual leaders as appropriate to ensure that our patients receive the best possible care and support. As a part of the hospice clinical team, you will use your clinical knowledge of hospice to maintain our Medicare and ACHC requirements with all care. This is a community-based, 100% in-person position, where you will be conducting in-home visits for much of your time. In addition to hospice patients, you will also be serving the spiritual health needs of our Palliative Care patients, and attendance at IDG meetings is required. You will be leading Grief Groups in person weekly as well as organizing and facilitating grief groups in our community facilities. Additionally, your certification in grief and bereavement will provide you with the skills needed to provide in-person, in-office visits with our bereaved clients and utilize EMR to chart all visits. Requirements

  • Bachelor's Degree in a related area, such as licensed therapy, social work, divinity, or theology required.
  • Certification in grief counseling or specialized certification in grief and bereavement required.
  • Current valid Washington State automobile driver's license and proof of automobile insurance.
  • Must be accepting of different lifestyles, cultures, beliefs, and values.

Nice To Haves

  • Master's Degree preferred.
  • Two years of experience working in a hospice program as a bereavement/grief counselor, chaplain, medical social worker, or counselor preferred.
  • Experience leading/facilitating support groups preferred.
  • Electronic Medical Record experience preferred, particularly with Epic.
  • Experience and ease with technology required.

Responsibilities
