Requirements

  • Graduation from an accredited four-year college or university with major course work in a related field, and experience in counseling and/or spiritual care, preferably in a hospice or health care environment; or any combination of training and experience that provides the knowledge, skills to perform essential functions of position.
  • Four completed units of Clinical Pastoral Education (CPE) or equivalent clinical training and experience
  • Ability to provide care in diverse living situations, to diverse patient population (racial, ethnic, religious, sexual orientation, age, social class).
  • Ability to work independently and with a transdisciplinary team.
  • Ability to communicate effectively, both orally and in writing
  • Ability to drive and have a valid driver’s license, car available and insurance coverage as required in Ohio
  • Demonstrated planning, follow-up and organization skills
  • Must demonstrate strong customer service and communication skills
  • Flexible to work both independently and in a team environment
  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
  • Provide an active driver’s license
  • Ability to provide proof of eligibility to work in the United States.
  • High level of integrity, ethics and professionalism.

Nice To Haves

  • Master of Divinity preferred
  • Considerable experience working with the terminally ill in a health care environment, program development, coordination of volunteers and grief counseling preferred

Responsibilities

  • Promote the philosophy and mission of Hospice of the Western Reserve within the transdisciplinary team including Spiritual Care Volunteers and patients/families that receive our services
  • Assess spiritual needs, provide direct consultation and develop spiritual care goals with the hospice patient/family and the hospice transdisciplinary team
  • Link hospice patient/family with appropriate community clergy or pastoral care visitors; collaborate with Coordinator of Volunteers with assigning the Spiritual Care Volunteers to patients and provide clinical supervision
  • Act as liaison between Spiritual Care volunteers, community clergy and the transdisciplinary team relaying pertinent patient/family information in a timely manner and encouraging participation in Team Conferences.
  • Assist the hospice transdisciplinary team in understanding ethical and religious/spiritual issues related to terminal illness, impending loss and bereavement
  • Participate in regular meetings with Spiritual Care volunteers to discuss relevant issues, provide peer support, obtain feedback regarding interventions with patients/families and assess and meet ongoing educational needs
  • Assist with the orientation, training, support initiatives and supervision of Spiritual Care volunteers including documentation process
  • Plan and assist with Memorial Services as requested by family or alternative home
  • Collaborate with Bereavement Coordinators in planning and implementing the hospice Memorial Services
  • Be available for emergency situations
  • Be flexible in meeting patient/family needs or speaking requests that require evenings and weekends
  • Participate in planning and implementation of an annual continuing education program to assist in further development of spiritual awareness within the transdisciplinary team
  • Maintain and complete all required documentation and reports according to agency standards
  • Adhere to the organizational policy, procedures and standards
