About The Position
Concierge Home Care is hiring Home Health Speech Therapists (SLP). Coverage Area: Port Orange FL / Volusia County (Territories designed to minimize drive time with clustered scheduling)
Requirements
- Active, unencumbered Florida Speech Language Pathologist license
- Valid driver’s license, reliable transportation, and auto insurance
- Home health experience preferred, but not required
Responsibilities
- Provide speech therapy services to patients in their homes on an intermittent basis.
- Conduct thorough assessments to evaluate speech, language, and swallowing conditions.
- Develop personalized, goal-oriented treatment plans in collaboration with patients and caregivers.
- Deliver therapy based on physician orders and care plans under the supervision of the Director of Clinical Services or Clinical Manager.
- Educate patients and caregivers on strategies to improve communication and swallowing function
- Complete documentation using streamlined AI-assisted tools
