About The Position

Crouse Community Center is seeking a compassionate and skilled Per Diem Speech Therapist to join our rehabilitation team. This flexible role is perfect for an ST who enjoys meaningful clinical work while supporting residents in a warm, collaborative long-term care environment. As a Per Diem Speech Therapist, you will provide high-quality evaluation and treatment services to residents with communication, cognitive-linguistic, and swallowing needs. You will work closely with nursing, medical staff, and interdisciplinary team members to ensure individualized, person-centered care.

Requirements

Current NYS Speech-Language Pathology license in good standing.
Master’s Degree in Speech-Language Pathology from an accredited program.
Strong communication, clinical reasoning, and documentation skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Professional, positive, and resident-focused demeanor.

Nice To Haves

Experience in long-term care or a similar healthcare setting preferred.

Responsibilities

Provide ST evaluations and treatments for communication disorders, cognitive-linguistic needs, and dysphagia.
Develop individualized treatment plans based on resident assessments and clinical findings.
Document progress and assessments in accordance with regulatory and facility standards.
Collaborate with interdisciplinary teams including nursing, medical staff, PT/OT, and social work.
Participate in MDS and care plan processes through timely communication and quarterly reviews.
Ensure safe therapy practices including adherence to infection control and PPE protocols.
Maintain a professional, supportive environment for residents and families.
