Speech Therapist (ST) Home Health (Per diem)
About The Position
Health Link Home Health Agency is seeking a compassionate, organized, and team-oriented Licensed Speech Therapist for per-diem home health work in the city and county of San Francisco. The therapist evaluates patients’ speech therapy needs and provides care according to each patient’s individual plan of care. The role works in coordination with the Director of Nursing, clinical staff, front office team, and interdisciplinary team to support coordinated patient care and improve patients’ health and quality of life. A dedicated back-office team supports field clinicians, and charting is completed online in Kinnser using company-provided devices. The position requires travel within a 15- to 30-mile radius and is explicitly not a contract position.
Requirements
- One year of previous Speech Therapy experience.
- Current Speech Language Pathology (Speech Therapist) license.
- Current California Driver’s License.
- Ability to travel within a 15- to 30-mile radius.
Nice To Haves
- One year of home health experience; preferred but not required.
Responsibilities
- Provide services within the scope of speech therapy, in accordance with the plan of care and in coordination with the interdisciplinary team.
- Follow the plan of care and provide treatment.
- Evaluate patients’ reactions to treatment and report changes in their condition to the Case Manager and/or Director of Nursing.
- Complete online charting in Kinnser in a timely manner using company-provided devices.
- Follow all company policies and procedures.
- Perform other duties as assigned.
