About The Position
The Visiting Nurse Association of Florida (VNA) is a 50-year-old home health agency seeking Speech Therapists across Florida. In this role, you will deliver quality speech therapy services, provide case management through comprehensive assessments, and coordinate agency services for patients. You will collaborate with family members and local healthcare professionals to implement services tailored to patients' and families' needs in their homes. Responsibilities also include completing physical assessments to determine patient home care needs and working with the care team using VNA of Florida's case management model.
Responsibilities
- Ensure delivery of quality speech therapy services
- Provide case management for patients through comprehensive assessments and coordination of agency services
- Work with family members and local healthcare professionals to implement services to meet the needs of patients and families in their homes
- Complete physical assessments and determine any patient home care needs
- Work with the care team for each patient while using VNA of Florida's case management model
