About The Position

Human Touch Home Health is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our growing team serving patients throughout Washington, DC. As a Home Health Speech-Language Pathologist, you'll help patients overcome challenges related to speech, language, cognition, communication, and swallowing while restoring confidence and improving their quality of life in the comfort of their own homes. Requirements

Active Washington, DC Speech-Language Pathologist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment, Communication, and Documentation Skills

Nice To Haves

Certificate of Clinical Competence (CCC-SLP) Preferred
Home Health Experience Preferred but Not Required
OASIS Experience is a Plus
Passion for Delivering Exceptional Patient-Centered Care

Responsibilities

Complete comprehensive speech, language, cognitive, and swallowing evaluations
Develop individualized treatment plans based on patient goals and clinical needs
Provide skilled speech therapy services in patients' homes
Educate patients and caregivers on communication techniques, cognitive strategies, and safe swallowing practices
Monitor patient progress and adjust treatment plans as needed
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
