About The Position

The Speech Pathologist will travel throughout the Agency service area to provide skilled speech-language pathology services. This includes performing comprehensive evaluations, developing and implementing treatment plans, communicating effectively with patients and healthcare teams, providing education, and completing accurate clinical documentation. The role also involves participation in care planning, case conferences, and ensuring compliance with all applicable requirements. Proficiency with computers and electronic health record systems is necessary.

Requirements

  • Master's Degree in Speech and Language Pathology from an accredited college/university
  • Current State of Connecticut licensure to practice Speech Therapy
  • Minimum of three years Speech Therapy experience
  • Meets all State of Connecticut guidelines for licensed home care Speech Therapy services
  • Demonstrates good verbal and written communication skills
  • Demonstrates ability to work with interdisciplinary team
  • Follows standards of practice for Speech and Language Pathologist
  • Ability to demonstrate, teach and supervise
  • Certified in CPR

Responsibilities

  • Provide skilled speech-language pathology services throughout the Agency service area.
  • Perform comprehensive evaluations and ongoing reassessments of speech, hearing, communication, oral functioning, and swallowing function.
  • Develop, implement, and modify treatment plans in accordance with physician orders, the plan of care, professional standards, and Agency policies.
  • Safely position, transfer, and assist patients as needed using appropriate safe-handling techniques.
  • Communicate effectively with patients, families/caregivers, physicians, and the interdisciplinary team.
  • Provide patient, family/caregiver, and staff education.
  • Complete accurate and timely clinical documentation, including OASIS assessments.
  • Participate in care planning, case conferences, hospice IDG meetings, and required Agency activities.
  • Maintain confidentiality and comply with applicable safety, infection-control, privacy, and Agency requirements.
  • Use computers, electronic health-record systems, telephones, and therapeutic or adaptive equipment.
  • Arrange for equipment required to increase patient's function and independence.
  • Coordinate care with all members of the patient health care team.
  • Evaluate effectiveness and outcome of care.
  • Regularly evaluate patient's progress and discharge patient when goals attained.
  • Participate in discharge planning.
  • Supervise HHA when such personnel are participating in the patient's Speech Therapy regime.
  • Evaluate patient’s hearing ability and tests hearing and recommends mechanisms to enhance patient’s hearing ability.
  • Record all necessary assessments, plans and visit data in a timely manner (within 72 hrs).
  • Report to physician and others as required in policies and procedures.
  • Complete initial evaluation within 24 hours of evaluation, including medical diagnosis, functional diagnosis, findings, problem identification, patient safety, goal setting, treatment provisions, rehab potential, physician communication, and comprehensive medication review.
  • Complete progress notes for every visit including findings, problems, progress, treatment provided, equipment ordered/obtained, patient/family education, home program, discharge planning, and goals for further sessions.
  • Document patient response to therapy intervention.
  • Complete a ten-day, sixty-day, and discharge summary.
  • Communicate with any member of patient care team.
  • Identify patient/family needs for other home health services and refer for same when needed.
  • Participate in case conferences, case management, team meetings and other meetings as necessary.
  • Participate in inservices and provides appropriate documentation of same.
  • Provide in-services as requested.
  • Attend staff meetings as scheduled.
  • Attend yearly OSHA in-service.
  • Identify knowledge gaps and remedies this independently.
  • Attend minimum 12 hours of in-services per year, with 6 hours on 'end of life care' issues for hospice patients.
  • Share information with other Agency personnel.
  • Be certified in CPR.
  • Participate in peer consultation and record review quarterly and QI process as appropriate.
  • Provide consultation to Agency staff, the community, and physicians as needed.
  • Maintain staff and patient confidentiality.
  • Abide by related Agency policies and appropriate departmental procedures.
  • Perform other appropriate duties as required by VNHFC.
