About The Position

Elara Caring is seeking a compassionate Speech-Language Pathologist to provide home health speech therapy. The role evaluates patients, develops individualized care plans, delivers therapy in accordance with the care plan and physician orders, and coordinates care with physicians, caregivers, and other healthcare professionals. The position involves travel between patient homes and assigned work locations and may require lifting or transferring patients and moving equipment.

Requirements

• Master’s degree from an accredited educational program in Language Pathology.
• Current, unrestricted SLP license in the state of work.
• At least one year of experience as an SLP in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Evaluate patients and create personalized speech therapy care plans.
• Complete initial and ongoing patient assessments in accordance with regulatory guidelines.
• Develop plans to help keep patients safe using available family and community resources.
• Communicate patient progress and changes with physicians and the care team.
• Deliver speech therapy services according to the patient's care plan and physician orders.
• Update care plans and request additional visits when needed.
• Provide education and home therapy exercises to patients and caregivers.
• Maintain accurate documentation and coordinate care with other healthcare professionals.
• Complete therapy notes and visit documentation on time.
• Lift, transfer, or otherwise physically support patients and move equipment as needed.
