PRN Home Health Speech Language Pathologist

About The Position
Elara Caring is seeking a compassionate Speech-Language Pathologist to provide home health speech therapy. The role evaluates patients, develops personalized care plans, delivers therapy in accordance with physician orders, and coordinates care with patients, caregivers, physicians, and other healthcare professionals. The position involves travel between patient homes and assigned work locations, as well as physical activity that may include lifting and transferring patients.
Requirements
- Master’s degree from an accredited educational program in Language Pathology.
- Current, unrestricted SLP license in the state of work.
- At least one year of experience as an SLP in a clinical care setting.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
Responsibilities
- Evaluate patients and create personalized speech therapy care plans.
- Complete initial and ongoing patient assessments in accordance with regulatory guidelines.
- Develop plans to help keep patients safe using available family and community resources.
- Communicate patient progress and changes with physicians and the care team.
- Deliver speech therapy services according to patient care plans and physician orders.
- Update care plans and request additional visits when needed.
- Educate patients and caregivers and provide home therapy exercises.
- Maintain accurate documentation and coordinate care with other healthcare professionals.
- Complete therapy notes and visit documentation on time.
- Perform physically active work that may include lifting and transferring patients.
