About The Position

Concept Rehab, a therapist-owned company with over 45 years of experience, is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) for a PRN opportunity in post-acute rehabilitation. The SLP will help patients improve communication, cognition, and swallowing while working collaboratively within an interdisciplinary rehabilitation team. The role involves evaluating patients, developing individualized treatment plans, providing skilled services, educating patients and caregivers, maintaining documentation, and participating in care planning to maximize patient independence and function.

Requirements

Master's degree in Speech-Language Pathology from an accredited program.
Current Speech-Language Pathologist license in the State of Michigan.
Strong communication, organization, and interpersonal skills.

Nice To Haves

Certificate of Clinical Competence (CCC-SLP) is preferred.
Clinical Fellowship candidates are encouraged to apply where applicable.
Experience in post-acute rehabilitation, rehabilitation services, or geriatric care is preferred but not required.

Responsibilities

Evaluate patients and develop individualized treatment plans for speech, language, cognitive, communication, and swallowing disorders.
Provide skilled speech-language pathology services in a post-acute rehabilitation setting.
Collaborate with Occupational Therapists, Physical Therapists, nursing staff, physicians, and other healthcare professionals.
Educate patients and caregivers on treatment strategies and home programs.
Maintain accurate and timely clinical documentation in accordance with regulatory and company standards.
Participate in care planning to help patients achieve their highest level of independence and function.
