About The Position
Home Health Care, Inc. is seeking a Speech Language Pathologist (SLP) to serve as an In-Home Communication & Swallowing Specialist in the Twin Cities Metro Area. This full-time role offers autonomy, flexibility, and the opportunity to provide meaningful one-on-one care directly to patients in their homes. The organization has over 30 years of home care excellence and provides a collaborative, patient-centered team environment with strong clinical support.
Requirements
- Active Minnesota SLP license.
- Valid driver’s license and reliable transportation.
- Strong interest in independent, home-based care.
Nice To Haves
- EMR experience preferred.
Responsibilities
- Provide in-home speech, language, and swallowing therapy.
- Complete evaluations and develop individualized treatment plans.
- Deliver functional communication and feeding interventions.
- Educate patients and families to support progress and safety.
- Document care in EMR to support clinical outcomes.
