About The Position
Horizon Home Healthcare LLC is seeking a licensed Speech-Language Pathologist (SLP) to provide high-quality speech therapy services in the comfort of patients' homes in Montgomery County, Ohio, and surrounding areas. This part-time/PRN position offers flexibility, autonomy, and the opportunity for one-on-one patient care, making a meaningful difference in patients' lives.
Requirements
- Current Ohio SLP license
- Current CPR and/or BLS certification
- Valid driver's license, reliable transportation, and auto insurance
- Excellent communication and organizational skills
Nice To Haves
- Home health experience is preferred
Responsibilities
- Evaluate and treat speech, language, cognitive, voice, and swallowing disorders
- Develop individualized treatment plans based on patient goals
- Educate patients and caregivers on treatment strategies and home programs
- Complete timely and accurate clinical documentation
- Collaborate with physicians, nurses, therapists, and other disciplines of the healthcare team
- Maintain compliance with state and federal home health regulations
