About The Position

The Speech-Language Pathologist (SLP) evaluates, plans, and provides speech therapy services to adult patients in their homes in accordance with physician orders and the Plan of Care (POC). Requirements

Active Speech-Language Pathologist license in the state of practice.
Master’s or Doctorate degree in Speech-Language Pathology.
Current BLS/CPR certification
Valid driver’s license, insured vehicle, and ability to travel to patient homes
Ability to work independently and manage schedule effectively
Experience working with adult/geriatric populations
Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

Responsibilities

Perform comprehensive speech, language, cognitive, and swallowing evaluations.
Assess the home environment and recommend adaptive equipment as needed.
Develop and update individualized Plans of Care in collaboration with the physician and care team.
Provide skilled speech therapy interventions using evidence-based techniques.
Monitor patient progress and modify treatment plans accordingly.
Document visits accurately and timely in the electronic medical record.
Communicate patient status to physicians, case managers, and interdisciplinary team members.
Educate patients, families, and caregivers on therapy goals and home programs.
Participate in case conferences, in-services, and team meetings.
Follow infection control and HIPAA privacy standards.
