About The Position
Always Best Care is a growing, Honolulu-based Medicare-certified Home Health agency serving patients across Oahu. Our mission is to help kupuna and local families remain safe, independent, and thriving in the comfort of their homes. We are seeking a compassionate and motivated Speech-Language Pathologist (SLP) to provide skilled in-home speech therapy services to adult and geriatric patients throughout Oahu.
Requirements
- Current Hawaii license as a Speech-Language Pathologist (SLP)
- 1+ year of clinical experience
- Strong communication and organizational skills
- Ability to work independently in the field
- Valid driver's license and reliable transportation
- Passion for serving seniors, kupuna, and local families
Nice To Haves
- Certificate of Clinical Competence (CCC-SLP) preferred
- home health experience preferred
- Knowledge of Medicare home health documentation requirements preferred
Responsibilities
- Provide skilled home health speech therapy services in patients' homes
- Complete speech-language pathology evaluations and assessments
- Develop and implement individualized treatment plans
- Treat swallowing disorders (dysphagia), cognitive-communication deficits, speech and language impairments, and voice disorders
- Educate patients, caregivers, and family members
- Collaborate with nurses, physical therapists, occupational therapists, and case managers
- Maintain accurate documentation in accordance with Medicare home health regulations
