About The Position
Speech Language Pathologist (SLP) at Tice Valley Post Acute. This role involves helping patients regain communication, cognitive, and swallowing abilities. Requirements
- Graduate of an accredited Speech Language Pathology program
- Current state Speech Language Pathologist license or eligibility to obtain licensure
Nice To Haves
- Certificate of Clinical Competence (CCC-SLP) preferred
Responsibilities
- Evaluate patients and develop individualized treatment plans based on clinical findings
- Provide speech, language, cognitive, voice, and swallowing therapy services
- Monitor patient progress and modify treatment plans as appropriate
- Maintain accurate, timely, and compliant documentation in accordance with facility, state, and federal guidelines
- Collaborate with physicians, nursing staff, therapists, and other interdisciplinary team members to coordinate patient care
- Educate patients and family members regarding treatment goals, progress, and home strategies
- Participate in care conferences, discharge planning, and interdisciplinary meetings
- Ensure compliance with all applicable state, federal, and company policies and procedures
