About The Position

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist for a per diem opportunity performing home health visits through its Montgomery County Visits Office. The office serves adult and geriatric patients in homes throughout Frederick County, Maryland. Prior home care experience is a plus but is not required. The position is part of a multidisciplinary health care team providing rehabilitative care and skilled nursing to clients so they can receive medical care at home.

Requirements

• Hold a current Speech Language Pathologist license in Maryland.
• Possess a Certificate of Clinical Competence in Speech from the American Speech-Language-Hearing Association; or meet the equivalent education and work experience requirements and be awaiting the opportunity to take the certification examination; or have completed the required education and work requirements and be accumulating the supervised work experience required for certification.
• Have solid computer skills.

Nice To Haves

• Prior home care experience as a Speech Language Pathologist.
• Prior experience with electronic medical records (EMR).

Responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate clients experiencing speech, language, communication, or swallowing difficulties; diagnose needs and provide therapeutic interventions.
• Develop treatment plans based on evaluations, physician orders, and recommended goals.
• Continually assess and revise speech therapy care plans and participate with BAYADA clinicians in multidisciplinary care planning, as appropriate.
• Educate and instruct patients, family members, or other representatives in rehabilitative care and activities that support health, safety, and independent living.
• Document observations, interventions, evaluations, patient care management, and services provided using a touch pad tablet.
