About The Position

Collaborative Health Care Services LLC is a nurse-owned and operated transitional home care agency seeking an experienced and dedicated Home Health Speech Language Pathologist for per diem work. The agency focuses on transitioning patients from hospital/facility to home and is a preferred provider for several local hospitals/facilities and Assisted Living Facilities. They emphasize a patient-centered approach and a supportive team environment.

Requirements

Experience in conducting patient assessments and developing individualized treatment plans
Familiarity with medical terminology and documentation practices
Ability to work effectively in a multidisciplinary team environment
Strong communication skills, both verbal and written
Experience working with diverse patient populations, including pediatrics and critical care patients
Master's degree in Speech-Language Pathology from an accredited program
Valid state license or certification in Speech-Language Pathology
Knowledge of human anatomy, physiology, and neurology related to speech and language function

Responsibilities

Conduct evaluations and assessments of patients to determine speech and language disorders
Develop and implement individualized treatment plans for patients with speech and language disorders
Provide speech therapy services to patients with various conditions, such as neurological disorders, developmental delays, or communication impairments
Utilize a variety of techniques and modalities to improve speech, language, and swallowing abilities
Collaborate with other healthcare professionals to coordinate patient care and ensure optimal outcomes
Maintain accurate and up-to-date documentation of patient progress and treatment plans
Provide education and support to patients, families, and caregivers regarding communication strategies and techniques
