VNA & Hospice of the Southwest Region of Rutland, VT is looking to hire a per diem Speech Language Pathologist for our Kids on the Move program in Bennington County. Requirements

  • Master's degree in Speech Language Pathology required
  • Current Vermont State SLP license required
  • Two years of clinical Speech Language Pathology experience required
  • Active enrollment in clinical fellowship year or successful completion of certificate of clinical competency required
  • Valid Driver's License

Responsibilities

  • Evaluate, plan, and administer medically prescribed or educationally relevant speech therapy treatment to clients in their homes or students in their respective educational environment.
  • Provide clinical supervision of students and SLP's in CFY year and students.
  • Actively monitor and amend patient visit schedule to maximize productivity and patient need. Take action to account for new patients and changes in existing patient schedule, including but not limited to, calling patient to reschedule visits, reshuffling patient load to meet new demands, communicating recommended changes to supervisor, etc.
  • Perform comprehensive (OASIS and SLP) assessments of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation.
  • Develop a plan of care to promote optimal functional independence and safety as documented by clinical record.
  • Plan for and provide skilled speech-language therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient's status and progress on each visit and ensuring quality of care as documented in the clinical record.
  • As Care Manager and/or team member, coordinate patient care with other clinical team members, including but not limited to, physicians, nurses, care givers, other rehab services and students, ensuring optimal patient care and communication as noted in the clinical record.
  • Accurately and completely perform functions as related to school-based services, including, but not limited to, evaluations, documentation, development of Individual Educational Plan IEP's, participation in meetings, and provision of direct and consultative services.
  • Fill out and complete insurance paperwork.
  • As necessary, assess patients' needs for HCA services, develop and coordinate POC with patient/care giver, and provide on-going supervision.
  • Coordinate safe patient discharges and/or transfers (or upon knowledge of) and ensure all appropriate paperwork and patient information is processed.
  • Serve as a Home Care ST as required.
