About The Position

Rehab Without Walls Neuro Rehabilitation is redefining speech therapy by bringing outpatient-level care directly to individuals in their homes and communities. Our Home & Community–based care model allows clients to practice communication, cognitive, and swallowing skills in the environments where progress truly matters. We are seeking an experienced Speech-Language Pathologist for a flexible PRN, W2 role in San Francisco, CA. This opportunity offers meaningful autonomy, strong clinical support, and the chance to provide high-impact, real-world care. We are looking for an experienced SLP interested in serving as the primary therapist throughout the full episode of care from evaluation through discharge, available for approximately 10–20 hours per week, typically serving 2–3 consistent clients. The ideal candidate is passionate about functional, real-world rehabilitation delivered beyond traditional clinic walls and ready to support clients as they transition back to meaningful daily activities. This role is for a clinician who thrives with professional autonomy while valuing collaboration with an interdisciplinary team of OTs, PTs, LCSWs, and other rehabilitation professionals. Rehab Without Walls® doesn’t limit therapy to staged environments. By bringing the outpatient experience directly to our clients, we help ensure carryover, relevance, and stronger functional outcomes. This role offers organizational and clinical support, payroll consistency and liability coverage, access to education, leadership, and collaborative team resources, PRN flexibility with the stability of an established neurorehabilitation program.

Requirements

  • Minimum of a Master’s Degree from an accredited Speech Language Pathology program
  • Valid Speech Language Pathology license in the state(s) of practice
  • Current CPR Certification in accordance with state regulations
  • A minimum of one year’s work experience as a Speech Language Pathologist
  • Demonstrates knowledge of rehabilitation techniques related to complex neurological injury preferred
  • Communicates effectively and professionally in verbal and written interactions
  • Ability to lift 50 pounds
  • Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
  • Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
  • Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
  • A health screen or examination may be required prior to assignment and periodically thereafter, depending on specific location policy, local and state regulations to verify employee is physically capable of performing assigned duties with or without reasonable accommodations

Responsibilities

  • Perform evaluations and develop treatment plans of patients with speech, language, cognitive and swallowing disorders
  • Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
  • Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
  • Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
  • Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
