About The Position

Mission Valley Post Acute is seeking a compassionate, motivated, and skilled Speech-Language Pathologist (SLP) to join our interdisciplinary rehabilitation team. Requirements

  • Master's Degree in Speech-Language Pathology from an accredited program.
  • Current state Speech-Language Pathologist license in good standing.
  • Strong communication, documentation, and interdisciplinary collaboration skills.

Nice To Haves

  • Certificate of Clinical Competence (CCC-SLP) preferred; CFY candidates may be considered.
  • Experience in skilled nursing, post-acute care, or rehabilitation settings preferred.

Responsibilities

  • Evaluate residents to determine speech therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
  • Develop, implement, and update individualized treatment plans to improve communication, cognition, swallowing function, and overall independence.
  • Assist the nursing department with training of Restorative Aides.
  • Supervise Speech-Language Pathologist Clinical Fellows (CFY) and students in resident care and related activities in accordance with state practice regulations.
  • Communicate regularly with physicians, nursing staff, interdisciplinary team members, residents, and families regarding resident progress, goals, and discharge planning.
  • Participate in Resident Care Conferences, Utilization Review meetings, and Rehabilitation meetings as needed.
  • Provide in-service education and training programs for facility staff.
  • Complete timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.
  • Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.
  • Educate residents, families, and caregivers regarding treatment goals, home programs, maintenance strategies, safety techniques, and aspiration prevention.
  • Recommend and facilitate ordering of appropriate durable medical equipment and adaptive devices to support resident independence and safety.
  • Contribute to quality assurance initiatives, performance improvement programs, and rehabilitation program development.
  • Establish and maintain positive professional relationships with residents, families, staff, and healthcare providers.
  • Supervise Speech-Language Pathology Clinical Fellows (CFY) and student clinicians, when applicable.
