About The Position

Legacy Lifecare VNA and Hospice is a leader in providing compassionate and comprehensive care, valuing both resident care and employee appreciation. This role involves evaluating patients for speech and language disorders, initiating assessments, and developing individualized plans of care in collaboration with the physician and multidisciplinary care team. The position requires providing rehabilitative speech-language therapy services, implementing appropriate interventions, and educating patients, families, and caregivers on treatment procedures and supportive activities. Responsibilities include monitoring, documenting, and reporting patient progress, responses to therapy, and changes in condition using the agency's Problem Oriented Record (POR) and required clinical documentation systems. The role also involves coordinating care with healthcare team members, consulting with supervisors, and participating in case conferences, in-services, and ongoing professional development. Additionally, the Speech Language Pathologist will support patients and families in adapting to physical, mental, and social limitations by providing education, encouragement, and resources to promote independence and continuity of care. Maintaining compliance with agency policies, patient rights, confidentiality, infection control, safety standards, and emergency preparedness, while effectively managing workload, scheduling, and priorities, is also a key aspect of this position.

Requirements

  • Active, good-standing MA Speech-Language Pathologist license
  • 1-3 years of experience in geriatrics, long-term care, or short-term rehab
  • Ability to communicate fluently in English - both verbally and written

Responsibilities

  • Evaluate patients for speech and language disorders, initiate assessments upon referral, and develop individualized plans of care in collaboration with the physician and multidisciplinary care team.
  • Provide rehabilitative speech-language therapy services, carry out appropriate interventions, and educate patients, families, and caregivers on treatment procedures and supportive activities.
  • Monitor, document, and report patient progress, responses to therapy, and changes in condition using the agency's Problem Oriented Record (POR) and required clinical documentation systems.
