About The Position

Legacy Lifecare VNA and Hospice East is seeking a Speech Language Pathologist (SLP) for a supplemental income opportunity. This PRN position offers flexible scheduling and competitive per-visit reimbursement rates. The role involves evaluating patients, providing rehabilitative therapy, educating patients and families, documenting progress, and coordinating care within a multidisciplinary team. The SLP will also support patients and families in adapting to limitations and maintain compliance with agency policies and standards.

Requirements

  • Active, good-standing MA Speech-Language Pathologist license
  • 1-3 years of experience in geriatrics, long-term care, or short-term rehab
  • Maintains professional and educational competency in compliance with state and federal regulations
  • Physically able to perform job duties, including standing, sitting, walking, bending, squatting, and lifting/carrying up to 20 lbs unassisted and up to 50 lbs with assistance
  • Demonstrates strong interpersonal skills, effectively interacting with residents, families, visitors, government personnel, and interdisciplinary staff
  • Applicants for this position must be able to produce a negative drug test.

Responsibilities

  • Evaluate patients for speech and language disorders, initiate assessments upon referral, and develop individualized plans of care in collaboration with the physician and multidisciplinary care team.
  • Provide rehabilitative speech-language therapy services, carry out appropriate interventions, and educate patients, families, and caregivers on treatment procedures and supportive activities.
  • Monitor, document, and report patient progress, responses to therapy, and changes in condition using the agency’s Problem Oriented Record (POR) and required clinical documentation systems.
  • Coordinate care with healthcare team members, consult with supervisors, and participate in case conferences, in-services, and ongoing professional development.
  • Support patients and families in adapting to physical, mental, and social limitations by providing education, encouragement, and resources to promote independence and continuity of care.
  • Maintain compliance with agency policies, patient rights, confidentiality, infection control, safety standards, and emergency preparedness while managing workload, scheduling, and priorities effectively.
