About The Position

Legacy Lifecare VNA and Hospice East is seeking a Speech Language Pathologist (SLP) for a supplemental income opportunity. This PRN position offers flexible scheduling and competitive per-visit reimbursement rates. The role involves evaluating patients, providing rehabilitative therapy, educating patients and families, documenting progress, and coordinating care within a multidisciplinary team. The SLP will also support patients and families in adapting to limitations and maintain compliance with agency policies and standards.

Requirements

Active, good-standing MA Speech-Language Pathologist license
1-3 years of experience in geriatrics, long-term care, or short-term rehab
Maintains professional and educational competency in compliance with state and federal regulations
Physically able to perform job duties, including standing, sitting, walking, bending, squatting, and lifting/carrying up to 20 lbs unassisted and up to 50 lbs with assistance
Demonstrates strong interpersonal skills, effectively interacting with residents, families, visitors, government personnel, and interdisciplinary staff
Applicants for this position must be able to produce a negative drug test.

Responsibilities

Evaluate patients for speech and language disorders, initiate assessments upon referral, and develop individualized plans of care in collaboration with the physician and multidisciplinary care team.
Provide rehabilitative speech-language therapy services, carry out appropriate interventions, and educate patients, families, and caregivers on treatment procedures and supportive activities.
Monitor, document, and report patient progress, responses to therapy, and changes in condition using the agency’s Problem Oriented Record (POR) and required clinical documentation systems.
Coordinate care with healthcare team members, consult with supervisors, and participate in case conferences, in-services, and ongoing professional development.
Support patients and families in adapting to physical, mental, and social limitations by providing education, encouragement, and resources to promote independence and continuity of care.
Maintain compliance with agency policies, patient rights, confidentiality, infection control, safety standards, and emergency preparedness while managing workload, scheduling, and priorities effectively.
