Requirements

  • Active Colorado Speech-Language Pathologist (SLP) license
  • Strong clinical judgment in cognitive and swallowing disorders
  • Ability to work independently in a home-based setting
  • Clear, compassionate communication style
  • Reliable vehicle, valid driver's license, and auto insurance

Nice To Haves

  • CCC-SLP preferred
  • CFs may be considered depending on experience 👀 optional if you want to open pipeline
  • Home health or medical experience preferred (we will train the right candidate)

Responsibilities

  • Evaluate and treat patients with speech, language, cognitive, voice, and swallowing disorders
  • Develop and implement individualized, functional treatment plans
  • Provide dysphagia management and cognitive-communication therapy
  • Educate patients and caregivers on strategies to improve communication, safety, and independence
  • Collaborate with an interdisciplinary team (RNs, PTs, OTs, social workers, caregivers)
  • Complete documentation in Homecare Homebase (HCHB) – training provided
  • Travel locally within Grand Junction, Fruita, Clifton, and Palisade
