About The Position

Hemet Hills Post Acute is seeking a passionate Speech-Language Pathologist (SLP) to join their therapy team. The SLP will be responsible for evaluating and treating residents with various disorders, developing individualized treatment plans, and collaborating with a multidisciplinary team to ensure comprehensive care. The role involves monitoring resident progress, educating stakeholders, and completing accurate documentation. Requirements

  • Current California Speech-Language Pathologist license
  • Strong clinical, communication, and documentation skills
  • Compassionate, resident-centered approach to care
  • Ability to work effectively within an interdisciplinary team

Nice To Haves

  • Skilled nursing or post-acute care experience preferred, but not required

Responsibilities

  • Evaluate and treat residents with speech, language, cognitive, communication, voice, and swallowing disorders
  • Develop and implement individualized treatment plans to support recovery and maximize independence
  • Collaborate with physicians, nursing staff, therapists, families, and other healthcare professionals to ensure comprehensive care
  • Monitor resident progress and adjust treatment plans as needed
  • Educate residents, families, and staff regarding communication strategies and treatment goals
  • Complete timely and accurate documentation in accordance with regulatory and facility requirements
