We are seeking a PRN Speech Language Pathologist to serve patients in the Middlebury and Danbury area. You will have the opportunity to provide personalized care while working with a collaborative and supportive team.

Requirements

Master’s degree in Speech Language Pathology
Current and unrestricted SLP license in the state of practice
At least one year of clinical experience as a Speech Language Pathologist
Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
Ability to travel throughout the Middlebury and Danbury coverage area
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Responsibilities

Evaluate patients and develop individualized plans of care.
Provide speech and language therapy services according to physician orders.
Help patients improve their communication, cognitive, and swallowing abilities.
Educate patients and caregivers on home therapy programs.
Maintain accurate and timely clinical documentation.
Follow OASIS, PPS, and company guidelines.
Collaborate with physicians and interdisciplinary team members to ensure continuity of care.
