About The Position

Concierge Home Care is hiring a Home Hospital Speech-Language Pathologist (SLP). This role offers flexibility, meaningful patient relationships, and the ability to control income. The coverage area includes Jacksonville, FL, and surrounding counties (Duval, Nassau, and Clay County), with territories designed to minimize drive time through clustered scheduling. This specialized division does not require Medicare OASIS documentation, allowing for a greater focus on patient care and clinical treatment.

Requirements

• Active, unencumbered Florida Speech-Language Pathologist license
• Valid driver’s license, reliable transportation, and auto insurance
• Employment is contingent upon successful completion of an AHCA Level 2 background screening.

Nice To Haves

• Home health experience preferred but not required

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans based on physician orders
• Provide therapy for communication, cognition, and dysphagia
• Educate patients and caregivers to improve functional outcomes
• Collaborate with physicians and interdisciplinary teams
• Document visits using streamlined, AI-assisted technology
