About The Position

Mobile Health Team Inc. is seeking an experienced Speech and Language Pathologists (SLP) - Long Term Care (LTC) for a 13-week travel assignment in Lewistown, MO. The role involves screening, evaluating, and rehabilitating patients with communication, cognitive, or swallowing disorders. The SLP will develop appropriate plans of care, utilize standardized assessments, track patient progress, and provide consultation to patients, families, and other service providers. Responsibilities also include documenting all required information in the electronic system, participating in meetings and training, obtaining necessary equipment, and potentially supervising CFY or student SLPs. Adherence to ASHA and State practice acts is essential.

Requirements

  • 1 year of SLP experience is required
  • New Grads OKAY- as long as a Level II experience and not just a Level I observation experience
  • LTC or Rehab experience is required
  • MO State License is required
  • CCC certified SLP is required
  • Missouri State License Requirements is required
  • Active ASHA and State Licensures is required
  • Excellent problem-solving and critical thinking skills
  • Strong organizational skills and ability to multitask
  • Willingness to participate in continuing education and training programs
  • Strong attention to detail and ability to work independently
  • Strong communication, professionalism, and reliability
  • Drug screen and background check (facility required)
  • Additional facility onboarding forms as assigned

Nice To Haves

  • 3 years experience holding active CCS is preferred
  • Will accept one Reference at time of submission

Responsibilities

  • Effectively screens/evaluates/rehabilitate patients with communication, cognitive or swallowing disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the speech disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy
  • Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
  • Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
  • Participate in facility and company required training, in-services and conferences.
  • Able to obtain any necessary DME or supplies per regulatory and clinical practice requirements.
  • Able to supervise and support CFY or Student Speech-Language Pathologists
  • Adherence to ASHA and State Required practice acts, code of conduct, ethics during professional practice as a Speech-Language Pathologist.
