About The Position

The Specialty Patient Navigator ensures psychosocial assessments are completed. This position facilitates patient-physician relationship and coordination of care across the spectrum of cancer services, including Palliative Services, and provides comprehensive, multi-disciplinary care to improve quality of life. The navigator works closely with Altru’s Cancer Committee and Cancer Center staff to evaluate data and assists with integration of identified relevant barriers into Quality Improvement Plan. The navigator participates in annual evaluation of Cancer Navigation program, and participates in a weekly multidisciplinary team meetings.

Requirements

  • Registered Nurse | North Dakota Board of Nursing (NDBON) or Compact State Agreement | Prior to Start Date | HR Primary Sources
  • Registered Nurse | Minnesota Board of Nursing | Within 3 Months of Start Date | HR Primary Sources
  • Basic Life Support (BLS) | American Heart Association (AHA) | Within 90 Days of Start Date | Learning Management System
  • A minimum of 1 year Related Experience
  • Proficiency in reading, writing, and speaking English to ensure effective communication in the workplace and with patients, families, and team members.

Responsibilities

  • Meets with new patients and arranges follow-up contact as appropriate for patient and family needs.
  • Ensures that patient psychosocial assessment is completed and identified needs are addressed with appropriate interventions and follow-up arranged.
  • Provides patient and/or patient’s family with appropriate information regarding the plan of care.
  • Acts as a resource for insurance and reimbursement needs.
  • Participates in the patient’s care through collaboration with physician and caregivers.
  • Communicates with referring facilities, or agencies including hospitals for coordination of care and relay of information, as needed.
  • Facilitates the patient-physician relationship and coordination of care across the spectrum of cancer services to provide comprehensive, multi-disciplinary quality care.
  • Collaborates with physicians and other members of cancer disciplines to improve quality of life for cancer patients and their families.
  • Works closely with staff to evaluate data and identify barriers patients experience to care.
  • Assists with integration of identified relevant barriers into Quality Improvement Plan.
  • Participates in an annual evaluation of Cancer Navigation program to foster improvements and enhance the program.
  • Assists with evaluation and selection of cancer center patient resources and programs.
  • Performs other duties as assigned or needed to meet the needs of the department/organization.
