About The Position

Under direct supervision, the Specialty Case Manager serves as a primary support and case manager for individuals with mental illness, assisting them to live as independently as possible in their chosen community. The position also provides knowledge of a specialty area to the treatment team and program. Telecare Early Intervention and Recovery (TEIR) provides services and supports at the facility and in the community for young adults aged 16–25 who may have emerging (prodromal) symptoms of psychosis, and their families.

Requirements

High school graduation or GED and three years of direct services experience in mental health or community services; or an associate degree and two years of experience; or a bachelor's degree in Social Services.
At least one year of experience in the area of specialty served, such as benefits, education, or employment.
Valid and current driver's license.
Personal vehicle insurance listing the employee as a driver.
Willingness to use a personal vehicle to drive clients to appointments and groups.
