About The Position

The IRIS Consultant (IC) partners with people enrolled in Wisconsin’s IRIS Program, a Medicaid long-term care option for older adults and people with disabilities. The position is remote and home-based, with regularly scheduled visits to participants in their homes and communities. ICs help participants navigate the program, identify long-term care goals, connect with community resources, and create customized plans addressing areas such as employment, housing, health, safety, community membership, transportation, and relationships. The IC provides program orientation, supports participants in exercising self-direction, monitors outcomes and budgets, documents activities and changes in the WISITs IRIS data system, and serves as a liaison between participants, Fiscal Employer Agencies, and the IRIS Consultant Agency. The role requires regular participant contact, collaboration with agency colleagues, and attendance at in-person team meetings. TMG describes its workforce as diverse and inclusive and says it prioritizes staff work/life balance and provides support from team members across the organization.
