About The Position

TMG is seeking an IRIS Consultant to work remotely from a home-based office and partner with people enrolled in Wisconsin’s IRIS Program and the TMG IRIS Consultant Agency. The position supports older adults and people with disabilities who use IRIS, a Medicaid long-term care option, by building relationships, identifying individual long-term care goals, and helping participants find ways to achieve them. Although the role is remote, it includes regularly scheduled visits with participants in their homes and communities, as well as in-person monthly team meetings. The consultant connects participants with community resources and helps them develop individualized IRIS plans addressing goals such as employment, housing, health, safety, community membership, transportation, and relationships. The role also includes program orientation, plan and budget support, documentation, coordination with fiscal and service partners, and ongoing communication about program requirements and changes. Consultants work independently while collaborating with their team and supervisor.

  • Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field; alternatively, four years of commensurate experience if no degree.
  • Valid and unrestricted driver’s license, reliable transportation, and adequate auto insurance for job-related travel unless otherwise required by law.
  • Ability to develop positive and effective working relationships with coworkers, clients, participants, providers, regulatory agencies, and vendors.

  • Excellent time-management and prioritization skills.
  • Ability to manage multiple projects simultaneously and adapt to change.
  • Ability to build and maintain professional relationships and work through challenging situations.
  • Comfort working in a variety of settings and adapting work style to diverse populations, personalities, and personal situations.
  • Demonstrated knowledge of community resources.
  • Proactive and detail-oriented approach.
  • Excellent verbal and written communication skills.
  • Proficiency with Microsoft Office Suite and applicable software programs.

Responsibilities

  • Meet in person with each IRIS participant at least four times per year, including one required annual home visit, and be available upon participant request.
  • Provide program orientation to new participants, including explanation of their rights and responsibilities, verification of legal documents, employee paperwork, and responsible use of public funds.
  • Explore participants’ goals, relationships, community connections, employment interests, awareness of the Self-Directed Personal Care option, and backup support plans.

  • Document progress and changes to participant plans and the data system when participants request modifications.
  • Prepare Budget Amendment or One-Time Expense paperwork when required by participant circumstances and budget needs.
  • Teach participants to read and interpret monthly budget reports and help them remain within budget.
  • Liaise between the Fiscal Employer Agency and the IRIS Consultant Agency, including assisting with provider billing, seeking support brokers, tracking receipts, ensuring workers are paid, and mitigating potential risks or conflicts of interest.
