About The Position

This role involves assisting others as a change agent and champion for new hospital and ICU-related initiatives, remaining readily available to assist others as needed, and adjusting workload and priorities based on patient needs. The Special Response Registered Nurse establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, families, and coworkers. They ensure the operation of equipment by completing preventive maintenance and calling for repairs, and maintain supplies inventory by checking stock, anticipating needs, and ordering. The nurse practices according to Franciscan Alliance and nursing division philosophies and standards of care, evaluating their own practice in relation to professional standards and guidelines. They assess, collect, and analyze comprehensive data pertinent to patient condition to determine diagnosis, needs, and plan of care, and respond to reassessments and care plan responses. Active rounding is performed to assess at-risk patients or any patients requested by nursing personnel. The role promotes patient independence by establishing care goals and teaching patients, friends, and families about their condition, medications, and self-care skills. A plan of care is developed based on data and continuously reviewed and revised as patient condition warrants. The nurse evaluates the individualized plan of care and progress towards outcomes in collaboration with the patient and family. They prioritize and implement the plan, coordinate care delivery, and employ interventions to improve patient outcomes while providing a safe, respectful, and ethical practice environment. A clean and safe work environment is maintained by handling and clearing instruments, equipment, and supplies according to infection-control policies. Medication is administered and stored according to controlled substance regulations. The nurse participates as an active member of the stroke team and attends stroke and chest pain meetings, as well as other patient care meetings as requested. They work in a highly dynamic environment and respond quickly to urgent calls in a calm manner. The role involves responding to all Code Blue and Rapid Response events, directing patient placement, and participating in or leading Mock Code Blue events to maintain staff readiness. Quality activities are participated in, and the nurse acts as a leader in service programs, a champion for rounding and service recovery, and is aware of coworker satisfaction and satisfaction of those served scores. Unit and SRN meetings are attended and participated in. Collaboration with patients, families, caregivers, and multidisciplinary team members ensures optimal patient outcomes and enhancement of the patient experience. Timely communication of changes in patient assessment and ongoing reassessment to providers is crucial, as is maintaining and coordinating communication with the multidisciplinary team for safe transitions and continuity of care. Continuity among nursing teams is maintained by documenting assessment, plan of care, interventions, and responses to care in the electronic medical record in a timely manner.

Requirements

Associates/RN Diploma- Required
2 years Critical care experience- Required
Cardiopulmonary Resuscitation (CPR) - Required
Registered Nurse (RN) - Required
Advanced Cardiovascular Life Support (ACLS) - American Heart Association-Required
Pediatric Advanced Life Support (PALS) - Required within 180 days
Stroke Certified - American Board of Neuroscience Nursing (ABNN)- Required within 180 days

Nice To Haves

Evaluates ones own nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules, and regulations.
Assesses, collects, and analyzes comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs and plan of care.
Responds to patient reassessments and responses to care plan.
Actively rounds to assess at risk patients or any patients requested by nursing personnel.
Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand the condition, medications, and self-care skills; answering questions.
Develops an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously reviews and revises the plan of care as patient condition warrants.
Evaluates the individualized plan of care and progress towards attainment of outcomes in collaboration with patient and family utilizing available data resources.
Prioritizes and implements the identified plan, coordinates care delivery, employs interventions to improve patient outcomes while providing a safe, respectful, and ethical practice environment.
Maintains a clean and safe work environment by handling and clearing instruments, equipment, and supplies in accordance with infection-control policies and protocols.
Administers and stores medication according to controlled substance regulations.
Participates as an active member of the stroke team.

Communicates timely changes in patient assessment, ongoing reassessment to providers that warrant consideration and or a change in the plan of care and maintains and coordinates communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery.
Maintains continuity among nursing teams by documenting the assessment, plan of care, interventions, and responses to care in the electronic medical record in a timely manner.
